Mitigation action taken by the facility to prevent/reduce the risk of transmission:

• Residents  with positive covid rapid tests, are transferred to the designated Covid Wing for Transmission Based Precautions.
• Attending Physician, Medical Director and Families are notified of the positive test and the transfers.
• Specimen for COVID PCR is sent to the lab to confirm the COVID rapid tests.
• Notify the Local Department of Health.
• Ongoing monitoring and surveillance of signs and symptoms for all residents and  dedicated staff.
• Ongoing  sanitization of the entire facility is being conducted.
• Adequate  supplies and appropriate use of Personal Protective Equipment (PPE) is  maintained at all times.
• Dedicated and adequate staffing to meet residents' needs.
• On-going Covid -19 rapid testing to residents and staff exposed to positive covid residents.
