Sub-Acute/Post-Hospital Therapy

Merry Heart’s Sub-Acute Program provides comprehensive inpatient care for individuals recently discharged from the hospital who need help with their recuperation. The primary goal of the Sub-Acute Program is to optimize functional ability, thus empowering our clients to return safely to their homes and families.

Each person is evaluated by Merry Heart’s highly qualified team of physical, occupational and speech therapists, as well as the nursing staff. Together they design an individualized treatment plan that ensures a speedy recovery. Sub-Acute/Post Hospital Therapy is offered at Merry Heart Health Care Center in Succasunna.

Skilled Nursing/Long Term Care

Since 1956, when we began as a small, 12-bed convalescent home in Succasunna, Merry Heart has offered only the best in skilled nursing care. Our Skilled Nursing/Long Term Care Program provides 24/7 nursing care by a highly capable nursing staff. Each resident in this program receives a care plan designed to meet his or her particular needs. It includes meals planned by a Registered Dietitian, housekeeping services, an individualized activities program, on-call physician services, as well as spiritual support if requested, all in a safe and caring atmosphere. Skilled Nursing/Long Term Care is offered at Merry Heart Health Care Center in Succasunna.

Assisted Living

The Merry Heart Assisted Living facility in Succasunna offers an active lifestyle for independent seniors who need minimal help with daily routines. Residents of Merry Heart Assisted Living will have comfortable, spacious apartments, each with its own bedroom, bathroom, sitting area, and kitchen. Merry Heart Assisted Living residents will have numerous daily opportunities for activities and socialization. If at any time a resident requires additional care support, these services will also be available.

Home Care

Merry Heart offers highly skilled, reliable and compassionate nursing staff to provide personal care in the comfort of your home. Our Home Care Program is tailored to meet each client’s particular needs. Services include nursing care, assistance with light housekeeping and hygiene, as well as ancillary services such as companion care and transportation assistance.

Short Term/Respite Care

Respite Care provides a service whereby caregivers can entrust their loved ones to Merry Heart for a short period of time if the need arises. Respite clients enjoy the same high level of care, delicious meals and engaging activities as those who stay with us full time. Short Term/Respite Care is offered at all our facilities.

Dementia Care

Residents in this program enjoy a plan of care tailored to address their specific needs. Features of this program include personalized diets, appropriate activities and socialization in a safe and caring environment. Alzheimer’s/Dementia Care is offered at all our facilities.

Hospice Care

The Hospice Care Program provides physical, emotional and spiritual support to residents dealing with terminal illness. The focus of the Hospice Care Program is to maintain the optimal level of comfort for each client through palliative care and management of pain in a compassionate, encouraging and loving environment. This program is available at all our facilities.
